    The prevalence and the clinical characteristics of metabolic syndrome patients admitted to the cardiac care unit

    AbstractObjectivesMetabolic syndrome (MetS) is a group of multiple cardiovascular risk factors, including dysglycemia, central obesity, high cholesterol, and hypertension. Cardiovascular disease is one of the most common complications of MetS. Recent studies showed that prevalence of MetS among patients admitted with acute coronary syndrome was as high as 46%.DesignWe conducted a cross-sectional study of 203 patients at the two main hospitals in Ta’if, Saudi Arabia. Patients older than 18years who were admitted to the Cardiac Care Unit (CCU) between the months of August 2013 and June 2014 were asked to participate. MetS diagnosis was made based on the International Diabetes Federation definition.ResultsA total of 203 patients participated, with 59.1% male and 40.9% were female. The mean age was 60.9 years with a mean body mass index of 28.97kg/m2 and a mean waist circumference of 95.45cm. The prevalence of MetS was 47.8%, primarily among obese female patients who reported sedentary lifestyles. Additionally, MetS patients were more likely to be admitted with heart failure (p<0.05) and more likely to have moderate-to-severe left-ventricular hypertrophy (LVH; p<0.05) relative to non-MetS patients.ConclusionOf the patients admitted to the CCU, 47.8% had MetS, with those patients likely to be female and obese. Furthermore, MetS patients were more likely to be admitted with heart failure and suffer from moderate-to-severe LVH

The toolkit provides automated domain analysis tools together with PDDL learning capabilities. New optimal and suboptimal planners extend state-of-the-art technology. With the tool, domain experts assist solving hard combinatorial problems. Approximate or incremental solutions provided by the system are supervised. Intermediate results are accessible to improve domain modeling and to tune exploration in generating high quality plans, which, in turn, can be bootstrapped for domain inference

    Globalization, democratization, and the Arab uprising : the international factor in MENA's failed democratization

    What explains the almost negative impact of international factors on post-Uprising democratization prospects? This article compares the utility of rival “diffusionist” and neo-Gramscian political economy frames to explain this. Three international factors deter democratization. The failure of Western democracy promotion is rooted in the contradiction between the dominance of global finance capital and the norm of democratic equality; in the periphery, neo-liberalism is most compatible with hybrid regimes and, at best, “low intensity democracy.” In MENA, neo-liberalism generated a crony capitalism incompatible with democratization; while this also sparked the uprisings, these have failed to address class inequalities. Moreover at the normative level, MENA hosts the most credible counter-hegemonic ideologies; the brief peaking of democratic ideology in the region during the early uprisings soon declined amidst regional discourse wars. Non-democrats—coercive regime remnants and radical charismatic movements--were empowered by the competitive interference of rival powers in Uprising states. The collapse of many Uprising states amidst a struggle for power over the region left an environment uncongenial to democratization.PostprintPeer reviewe

    Islamism and the state after the Arab uprisings: Between people power and state power

    The authors would like to thank the Arts and Humanities Research Council for facilitating the research for this article through their support of the research network People Power versus State Power of the Centre for the Advanced Study of the Arab World.This paper examines the trajectories of different Islamist trends in the light of the Arab uprisings. It proposes a distinction between statist and non-statist Islamism to help understand the multiplicity of interactions between Islamists and the state, particularly after 2011. It is outlined how statist Islamists (Islamist parties principally) can contribute to the stabilization and democratization of the state when their interactions with other social and political actors facilitate consensus building in national politics. By contrast when these interactions are conflictual, it has a detrimental impact on both the statist Islamists, and the possibility of democratic politics at the national level. Non statist-Islamists (from quietist salafi to armed jihadi) who prioritize the religious community over national politics are directly impacted by the interactions between statist Islamists and the state, and generally tend to benefit from the failure to build a consensus over democratic national politics. Far more than nationally-grounded statist Islamists, non-statist Islamists shape and are shaped by the regional dynamics on the Arab uprisings and the international and transnational relations between the different countries and conflict areas of the Middle East. The Arab uprisings and their aftermath reshaped pre-existing national and international dynamics of confrontation and collaboration between Islamists and the state, and between statist and non-statists Islamists, for better (Tunisia) and for worse (Egypt).PostprintPeer reviewe

    COMMUNICATIVE BEHAVIOR IN THE DEVELOPMENT OF FRIENDSHIP: A CROSS-CULTURAL PERSPECTIVE

    This study explored the communicative behavior in the development of interpersonal relationships between and within cultures. The study began by mapping perspectives and approaches to development in human relationships. The connection between communicative behavior and interpersonal relationships was described, models of relational development were identified, and conceptualization of dimensions of communicative behavior in the development of interpersonal relationships was presented. The focus was on communicative behaviors in the development of relationships from acquaintanceships to close friendships between and within two cultures. The data were obtained from American and Middle Eastern subjects at an intra, cross, and projected cross-cultural level. Each subject completed six forms of a 30-item questionnaire, one form on each of the two relationship types at each of the three levels. Analysis of communicative behavior progressed in three phases: (1) an examination of the patterns which characterize the nature of Americans\u27 and Middle Easterners\u27 communicative behavior descriptive systems in the development of their intra, cross, and projected cross-cultural relationships; (2) an exploration of the dimensions of communicative behavior in relationship development at the three levels; and (3) an examination of the impact of culture and personal demographics on Americans\u27 and Middle Easterners\u27 communicative behaviors during the development of their relationships from acquaintanceships to close friendships. Results revealed differences between and within the American and Middle Eastern cultures. Americans and Middle Easterners perceived the dimensions and patterns of communicative behavior differently during the development of their relationships. Results also revealed that the generality of a useful, applicable intra-cultural system of communicative behavior is doubtful, while that of a cross or projected cross-cultural system is possible in the development of friendship relationships across cultures. Replication of this study with different populations, types of relationships, and settings; precise, objective operationalizations of variables; and conceptualizations of trans-cultural dimensions and factors were suggested for the formulation of a model of friendship development across cultures

    State trajectory and preference constraints are the two language features introduced in PDDL3 (Gerevini &amp; Long 2005) for describing benchmarks of the 5 th international planning competition. State trajectory constraints provide an important step of the agreed fragment of PDDL towards the description of temporal control knowledge and temporally extended goals They assert conditions that must be met during the execution of a plan and are often expressed using quantification over domain objects. We suggest to compile the state trajectory and preference constraints into PDDL2 (Edelkamp 2006). Trajectory constraints are compiled into Buchi automata that are synchronized with the exploration of the planning problem, while preference constraints are transformed into numerical fluents that are changed upon violation.     Retrovesical hydatid cyst presenting with urinary retention and left kidney atrophy

    Background: Hydatid disease of the urinary tract is uncommon, accounting for only 2-3% of cases. There are very few reported cases in the literature of pelvic hydatid cysts causing obstructive uropathy and renal failure. Materials and Methods: We report a case of pelvic hydatid cyst in a patient presenting with urinary retention and secondary complete atrophy of one kidney. Results: The patient was treated with surgical excision of this large retrovesical cyst, along with a simple left nephroureterectomy, with rapid improvement of symptoms. Conclusion: Hydatid disease should be taken into consideration in the differential diagnosis of a cystic mass in any anatomic localization, especially in patients from endemic areas
